Warranty Request Form

The Siemon Company ("Siemon") together with its program participants provide system performance assurance and product warranties for telecommunications cabling systems (the "Siemon Cabling System" or "System/s/") which utilize authorized Siemon Products and Qualified Cable and which are designed and installed by Authorized Designers and Installers in accordance with Siemon Specifications for the agreed upon period from the date of completion.

This form is a confirmation that prior to contract/project award, the Customer will review an overview of the Siemon Cabling System offerings and applicable warranties as presented by the Authorized Designer or Installer who, along with Siemon, will be bound by the provisions of the registered warranty.

The Siemon Company (Siemon) has invested a significant amount of time and resources in the design, development and implementation of its warranty programs. Therefore we ask that you respect our desire to keep the warranties confidential. The Customer is requested to select one of the Siemon Cabling System Warranties listed below. Siemon will then furnish the applicable warranty for the selected System for the customer's review prior to project award.
